Reproductive System

Male genital tract

An assembly line with one exit · Organa genitalia masculina
About 100 million sperm made a day, each taking some 70 daysevery single day

Testis, epididymis, glands and urethra — a continuous line that makes cells, matures them for weeks, and adds most of their fluid at the last moment.

Key facts

Size
Testis about 4.5 cm; the epididymal duct, uncoiled, about 6 m
Weight
Testis 15–25 g each; prostate about 20 g at twenty
Location
The testes outside the abdomen, the glands and ducts deep in the pelvis
Function
Makes sperm and testosterone, and delivers them

Did you knowSperm leave the testis unable to swim. They acquire motility while travelling the epididymis — a duct six metres long coiled into a structure the size of a thumbnail.

Clinical notes

Red flags

  • Sudden severe testicular pain, often with vomiting, is torsion — the testis is lost in hours, so it goes to theatre on the story rather than on a scan.
  • A painless lump arising from the testis itself is a tumour until proven otherwise, and it is the commonest cancer of young men.
  • A man who suddenly cannot pass urine at all needs the bladder drained before anything else is discussed.
  • Benign prostatic hyperplasia
  • Prostate cancer
  • Testicular torsion
  • Varicocele
  • Epididymo-orchitis
  • Testicular cancer
  • Erectile dysfunction
  • Male infertility
